Retzer speaks at Kiwanis Club

HURON The Kiwanis Club of Huron recently welcomed Miranda Retzer, RN, from the HRMC Physicians Clinic, to present on the clinics Chronic Care Management (CCM) program.

Retzer provided an insightful overview of how the program supports patients managing chronic health conditions, helping to improve their quality of life through personalized, ongoing care.

As a dedicated Chronic Care Management nurse, Retzer works one-on-one with patients to create individualized care plans, coordinate services, and provide ongoing support. Her role is to help patients navigate the challenges of living with chronic conditions and to make daily life a little easier.

To be eligible for the Chronic Care Management program, patients must:
Be a patient at either New Life Clinic or HRMC Physicians Clinic
Have Medicare
Have two or more of the following chronic (or other qualifying) conditions:
Alzheimers Disease
Arthritis
Cancer
Cardiovascular Disease
COPD
Diabetes
Hypertension or high blood pressure

Retzer emphasized that early and consistent care coordination can reduce hospitalizations, improve medication adherence, and empower patients to take an active role in their health journey.
