James Valley Community Health Center receives national care recognition

HURON — Horizon Health Care’s James Valley Community Health Center has been awarded recognition by the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home Program in recognition of delivering high-quality, patient-centered care.

The NCQA Patient-Centered Medical Home Recognition program is the most widely adopted in the United States with more than 10,000 practices and 95 organizations supporting the recognition. PCMH puts patients at the forefront of care and encourages the building of strong relationships between patients and their primary care providers. Research shows that this model creates an atmosphere that results in increased staff and patient satisfaction and improved quality of the medical visit, while reducing health care costs. Providers and health systems who earn this recognition have made an commitment to continuous quality improvement and patient-centered approach to care.

“With each patient encounter Horizon providers strive to create an atmosphere of comfort and confidence in their provider and caregiving team,” said Misty Rudebusch, PA-C, Horizon’s chief medical officer. “This recognition is a physical representation of our efforts to continually create better experiences for our patients.”

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others.

“Our continued recognition from NCQA and the PCMH program, is a testament to the hard work of our providers and staff who have created a home for their patients to come to when they need care,” Horizon CEO Wade Erickson said. “This is yet another example of what makes the Horizon Health Care family special.”

NCQA is a private, nonprofit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance. NCQA’s Healthcare Effectiveness Data and Information Set (HEDIS®) is the most widely used performance measurement tool in health care.
